And now I have a short story brewing in my head about a society that has men as doctors and women as surgeons. Except for our protagonist who wants to do both. And that's just clearly not okay with anyone else. Thanks.

They'd have to crossdress at least if I'm having them sneak into classes.
I have a short story too (see below the oath quote). It does not include women because the oath text does not either. You can rewrite it to include gender mechanics (making the patient a woman, perhaps) but it's probably not worth the effort. For starters, appendoctomy hadn't been invented back then.
“Could I fuck a female doctor instead?”
“To my knowledge, there aren’t any in all of Pelopones. There is quite a selection of gay men, though. A nearby one is crossdressing, actually.”
That's hilarious. Can't do it during office hours!
But yeah I tend to latch onto much smaller parts. So I'll probably run with the gender divide in a steam punk society where surgeon's and doctor's knowledge bases overlaps something like 80%. It's that last 20% they guard so fiercely. I think it'll start somewhere slightly funny like the protagonist is a student who realizes that doctor classes and surgeon classes aren't on the same days, so why not attend both?
